It looks like the emmc you have is not in good condition,
take a look at the health report in the log window,
it reads Device Lifetime = Urgent,
so you can conclude that emmc is no longer suitable for use.
Try replacing it with a normal emmc,
rewriting it using the factory image tab in the UFI eMMC ToolBox, and post the result here.
